Fisher & Paykel Forma Full Face CPAP Mask forms a superior seal and provides even more comfort with a greater range of movement through the active contouring of the new FlexiFoam Cushion. A new T-Piece adds stability to the mask base, and works in harmony with the highly contoured Silicone Seal and Under-Chin Deisgn to enhance sleep performance. With its ease of use and superior conforming seal, the new Forma Full Face CPAP mask sets a new standard.

The new Fisher & Paykel Forma mask offers an improved seal and a more comfortable fit due to the interaction between the new and improved FlexiFoam cushion and the patented Under-Chin Design (as see in the FlexiFit 431 and FlexiFit 432). The Forma Full Face CPAP mask comes full assembled and ready for use.

*Note: The Forma comes in 3 different sizes. The MEDIUM/LARGE size contains both LARGE and MEDIUM sizes, and the SMALL and EXTRA LARGE sizes contain one cushion.

FlexiFoam Active-Contouring Cushion - The Forma mask includes this new innovation from Fisher & Paykel. Greater comfort using a soft, lightweight and supportive foam that easily contours to many face shapes.
Unique Glider Strap with Easy Release Cord - Provides freedom of movement and one-step removal.
Under-Chin Design with Crown Strap - The patented Fisher & Paykel Under-Chin design provides extra stability and maintains your seal, even during movements of the jaw. The headgear also incorporates a Crown Strap design that eliminates the need for an additional chin strap.
T-Piece Mask Base - This ne feature on the Forma provides added stability and complements the provern Under-Chin design.
Advanced Air Diffuser - This maintenance free diffuser system prevents air from blowing onto your sleeping partner and decreases noise that could disrupt your sleep.
Frosted Silicone Seal - A soft and smooth feeling sealing cushion on the Fisher & Paykel Forma also incorporates a frosted texture to optimize your seal.

I've used this mask for 9 months and have had no problems. Once you get the fit set, it's off to dreamland for a good nights sleep and refreshed in the AM ready to start a new day. I've had no problems with the fit, the comfort, the stability of the mask what so ever. I would highly reccommend this to any CPAP user. It's easy to clean and reassemble and has shown no signs of breakdown or adverse wear.

This mask leaked and fell apart from the start. I required tightening until it left marks to get any kind of seal and then it was to uncomfterable to sleep in. This thing is a piece of junk.

I have had problems with leakage around/beside my nose. I have had to replace the foam cushion within a few days usage because it split at the thin nose part. Now the new one is splitting after only 2 weeks usage. I will be looking for a replacement mask soon.

I've been using this mask for 7 months now and I'm happy with it. At first it takes a few tries to get the fit exactly where you want it but after that its a very good mask. Its very comfortable and constructed very well. During my sleep study I tried 3 different mask and made sure when the Dr. ordered mine that he ordered this one. I would recommend this mask to anyone.
