The Philips-Respironics "System One" REMstar Auto with A-FLEX takes a smarter approach to the entire sleep management process. New features such as resistance control, advanced event detection and response, upgraded data management and reporting, A-FLEX, Dry Box technology, and System One humidity control put this CPAP machine on the cutting edge.

A-Flex is designed to work with Respironics’ clinically proven auto CPAP algorithm to improve comfort even more by matching pressure delivery to the patient’s entire breathing cycle. Like C-Flex, A-Flex provides flow-based pressure relief at the beginning of exhalation. Like Bi-Flex, A-Flex softens the pressure transition from inhalation to exhalation to provide additional comfort in an auto-CPAP mode. The pressure profile mirrors the patient’s breathing, and with the auto algorithm intelligence, it will respond to the patient’s therapeutic need throughout the sleep session.

The System One resistance control allows the patient to tell the machine what type of mask you are using such as a nasal, full face, or nasal pillow. The machine then makes adjustments for optimal flow depending on your mask. Each Respironics mask will now come with a resistance control setting letting you know how to set your machine to match your mask.

Philips-Respironics has upgraded their event detection algorithm to include data on Snoring, Periodic Breathing (PB), Respiratory Effort-Related Arousals (RERAs), Apnea Hypopnea Index (AHI), leak, Clear Airway Apnea (CA), Obstructed Airway Apnea (OA), Flow Limitation (FL), and Hypopnea (HY). The REMstar Auto will also record Patterns of use and Flow waveform data. With this information, your physician will be able to better determine the best possible solutions for your sleep therapy.

In addition to detecting the above events, the REMstar Pro will respond to FL, RERA, Snore, leak, OA, and HY. If the REMstar Pro detects that any of these events are about to occur, it will adjust pressure accordingly to compensate for the event.

The PR System One uses a SD card for data collection. Prior to the System One, patients were required to use a specialized card reader when uploading sleep data to a computer. Philips-Respironics has made this process easier by using the standard SD card, giving the patient the ability to use any SD reader to upload sleep data.

Philips-Respironics also listened to user feedback when designing the System One Heated Humidifier. A cleverly designed water chamber works with a built in Dry Box to virtually eliminate water getting into the machine during normal use. Even with a water chamber filled to the FULL line, the machine can be tilted and even turned upside down without leaking water back into the unit*. Furthermore, System One Humidity Control monitors room temperature, room humidity, and flow to determine what is required to maintain your selected humidity setting, even when environmental conditions change. Because of this, you can be assured that your therapy comfort will be maximized by virtually eliminating rain-out and condensation buildup in the tubing.

I had high hopes, but I think this machine is a step back from the previous model, the M Series. The new drybox technology makes the humidifier much larger without increasing the capacity. Even worse, the humidity control technology does prevent rain-out as advertised, but it does it by putting so little moisture into the air that I get bloody noses. The humidifier is easy to use, though. The machine itself has some fine technology in it, some of which results in short, very irritating bursts of air being produced periodically. Although most of the lights on the unit can be turned off, the humidifier light remains on all the time. If the light bothers you, this can be fixed with a little tape, but I just don't know why they thought I would want to turn off all of the lights but that one. The unit is plenty quiet and relatively portable, but larger than the unit it replaced. Software isn't available for this machine yet, so if you like monitoring your therapy closely, you're out of luck for now, as the 7-day averages don't tell you much. Between being dryed out and blasted with puffs of air, I haven't slept well with this machine. My advice is to buy an old M Series before they sell out and save some money.
